Instructions for Submitting a Request for Special Accommodations for the Personal/Biographical Questionnaire

The personal/biographical questionnaire is administered in a computer session at the NITE test centers. The test centers are accessible to people with various limitations and the tests are conducted in rooms containing a limited number of examinees, with comfortable seating arrangements (at desks).

Applicants entitled to submit a request for special test accommodations are those with a physical restriction who consider that it is liable to affect their ability to fill out the personal/biographical questionnaire and therefore require special accommodations, for example, breaks during the test or desks suitable for wheelchair access.

In order to submit a request for special accommodations for the personal/biographical questionnaire, register for the test using the registration link appearing on the website. After registration, you can log on to the website and submit requests. Note that registration for the questionnaire and submission of a request does not guarantee a test appointment. The selection of candidates is exclusively at the discretion of the teaching institutions. Examinees who are granted special accommodations and invited to fill out the questionnaire should allocate themselves to a test date from those appearing on the website.

As requested by the institutions, NITE will pass to the medical schools that an applicant has applied to, every piece of information relevant to his or her application to study medicine, including the documentation attached to an application for special accommodations. When registering for the test, you will be requested to sign a declaration confirming that you accept this.

Important points

  • Processing and verification of the documents accompanying your application can take up to 25 business days.
  • If you submit your request within a month before the registration deadline for submitting an application for special accommodations, as specified in the handbook for examinees, it is unlikely you will have enough time to submit additional documents or appeal the decision of the professional department. We recommend registering and submitting the application as early as possible.
  • If you have submitted documents for previous test dates, they are kept on file. Please do not send them again.
  • Note: There is a service fee for sending additional documents after you have already submitted your request - even if you have not yet received an answer - or for submitting an appeal.
  • The professional staff of the Special Test Accommodations Unit accepts documents in Hebrew, English, and Arabic. Documents in any other language must be translated into one of the three languages mentioned above and submitted along with the original documents. As customary, translations must be notarized and accompanied by a translator’s affidavit.

Instructions for Submitting an Application Due to Physical Restrictions

When submitting an application for accommodations on the basis of a physical restriction, the following should be attached:

  1. a current and detailed medical assessment, prepared by a physician who specializes in the relevant field (e.g. an orthopedic specialist must diagnose an orthopedic problem). Documents from a general practitioner or family doctor will not be accepted.
    The assessment must have been prepared within a year of the date of the test you are registered for, and must include all of the information listed in the questionnaire, as explained below.
  2. background documentation as detailed in the guidelines below.

Current and detailed medical assessment

The assessment must be submitted using the appropriate questionnaire:

If the physician is not willing to complete the questionnaire, ask for a written and signed document that addresses all the questions that appear on the questionnaire.

Background documentation

In cases of a physical restriction diagnosed more than a year ago, documentation of the existence of the condition in the past is required in addition to a current assessment. (There is no need to submit complete medical records.)

In cases of a newly acquired physical limitation, the doctor’s assessment must describe the cause of the problem (if possible). In such cases, NITE may request objective test results that attest to the existence of the restriction.

Note: The medical assessment provides information that is essential for determining accommodations for the test, but NITE is not obligated to grant any specific accommodations that it mentions. Should the doctor recommend a particular accommodation, they should explain why and support the recommendation with test results and specific findings.

How to Submit an Application for Special Accommodations?

Applications for special accommodations for filling out the personal/biographical questionnaire may be submitted by logging onto the NITE website according to the following instructions:

  1. First, register for the personal/biographical questionnaire using the link on the website.
  2. Only after receiving confirmation of registration, it will be possible to log on to the website and submit a request for special accommodations.

How to submit the application

After receiving confirmation of your registration for the test, log onto the website. Under “List of My Tests”, choose the personal/biographical questionnaire and click on the orange arrow on the right side to open the relevant list. On the left, there will be a tab labeled “Test with Special Accommodations” - click on that in order to start the process of submitting an application.

  1. Select the appropriate reason for application (physical restrictions)
  2. Click on the questionnaire, and fill it out.
  3. Attach all the required documents, as listed above.
  4. Confirm the details and submit the request.

After the request is received by the system, you will receive confirmation of this via SMS.

Please note: If you do not receive an SMS after submitting your request, log back onto the website and make sure you completed the process properly.

Important Instructions

  • The documents must be uploaded as PDF files, in their original size. They must be complete, clear, and legible and include all identifying information. Documents that do not meet these requirements will not be processed.

  • Up to 20 files can be uploaded, therefore, it is recommended that you scan each document separately. Try not to send each page as a separate file. If the system warns you that a file you have uploaded is too big, divide it into two parts; make a note in the file name (i.e., Part 1, Part 2); and upload each file separately.

Please note: We recommend that you save copies of all documents and questionnaires you submit to us in case you should need them in the future. For confidentiality and information security reasons, we cannot return documents to their senders.

Retaking a Test with Special Accommodations

The law prohibits NITE from assigning you to a test with special accommodations unless you submit an explicit request. This is true even if you received special accommodations previously.

Note! Even if you received special accommodations for another test at NITE, special accommodations for the personal/biographical questionnaire are only granted due to physical limitations.

It is important to make sure that your documentation is current, i.e., issued no more than a year before the date of the test for which you are registering. If the professional opinion you submitted previously was not issued in the past year, you will need to provide a new one.

If you received special accommodations for any test in the past, when registering, immediately before making payment the following options will be presented:

Do not choose “request special accommodations” or “provide additional documentation” unless you are requesting special accommodations due to physical limitations.

Reapplication on the Basis of Existing Documentation

If you select this option, the professional staff will review your request for special accommodations on the basis of documents already on file.
You will receive an SMS confirming receipt of your application.

Please note: If you do not receive an SMS after submitting your request, log back onto the website and make sure you completed the process properly.

Once you select this option, any appeal or submission of additional documents is subject to a fee.

Reapplication with Additional Documentation for Professional Staff Review

If you wish to receive additional accommodations or submit updated documents, you may submit additional documents for the professional staff to consider after you register for the test. To do so:

  • Log onto the website. Click on the test you registered for and then on “special accommodations” and follow the instructions. You will receive an SMS confirmation of receipt of your application.
    Please note: If you do not receive an SMS after submitting your request, log back onto the website and make sure you completed the process properly.
  • Do not attach documents that you have submitted in the past.
  • Your application will be reviewed based on the documents we have on file and additional ones that you attach.
  • Once you have submitted your application, submission of additional documents or lodging an appeal is subject to a fee.

Submission of Additional Documentation and Appeals

It is possible to submit additional documents or appeal NITE decisions up until the deadline for requesting special accommodations listed in the handbook for examinees.

Appealing a decision of the professional staff or submitting additional documentation for another review returns the case to the professional staff and hence is subject to a fee of NIS 265.

In order to submit additional documents or appeal, log onto the website. Select Personal/Biographical Questionnaire from the list of tests and click on “Special Test Accommodations”. If you have already applied for accommodations and registration for the test is not yet closed, you can appeal a decision of the professional staff or submit additional documentation. We recommend explaining your appeal in a personal letter.
After submitting the appeal or additional documentation, proceed to the payment link. If you do not pay, your request will not be processed.

You will receive an SMS confirmation when the re-evaluation of your case begins.
