The specialist’s assessment must be current, i.e. from within a year before the test you have registered for. It should be submitted using the appropriate questionnaire, as detailed below.

• Questionnaire for physician specializing in the restriction, on the grounds of a general medical or physical restriction, for those applying on the grounds of chronic diseases, mobility issues, temporary restrictions (such as fracture or injury) and other medical conditions
• Questionnaire for oculist, for those applying on the grounds of sight impairment
  Note: applicants with a certificate of blindness are not required to fill out the physician’s questionnaire, and need only submit their certificate.
• Questionnaire for ear, nose and throat doctor, for those applying on the grounds of hearing impairment.
  In cases of non-bilateral hearing impairment which manifested from a young age, you should also submit documents relating to educational function over the years, including a current assessment of educational functioning (as detailed here).

Pregnancy: a certificate from a gynecologist specifying the week of pregnancy when submitting the application and on the test date.

The physician should answer all the questions in the relevant questionnaire. If the physician is not willing to complete the questionnaire, you may submit a written and signed document, on condition that it addresses all the questions that appear on the questionnaire. The document should include the physician’s telephone number and office hours.

Note: the medical assessment provides essential information required for determining the special accommodations for the test. However NITE is not obliged to accept recommendations for specific accommodations that it includes. If the physician recommends a particular accommodation, you should ask him to justify his recommendation on the basis of tests and detailed findings.

In cases of a physical limitation diagnosed more than a year ago, medical documentation or other official documentation certifying the existence of said limitation in the past must be submitted, in addition to the current expert opinion. There is no need to submit complete medical records.

In cases of a recently acquired physical limitation, the doctor’s assessment must describe the cause of the problem (as far as possible). In such cases, NITE may request test results attesting to the existence of the restriction.

No. The physician’s recommendation is important information and we treat it seriously, but the decision on special accommodations is taken by NITE according to the complete medical information submitted in the request as well as psychometric considerations. So the decision may differ from the recommendation.

Orthopedic limitations require an assessment by an orthopedist, chronic diseases require an assessment by a doctor specializing in the area of the disease (endocrinologist, gastro-enterologist etc.) and so on. As assessment by a family doctor, pediatrician or general practitioner is not considered an adequate document for the purposes of submitting a request for special accommodations.

The following are the special accommodations normally granted for those possessing a certificate of blindness, according to the professional guidelines and the needs arising from the documentation:

• A pen and paper test with an accompanying audio system
• Escorting proctor, to operate the test or audio playback, mark the answers and write the writing assignment
• Large print test material (if the examinee can read unaided)
• Extra time
• Breaks

Additional aids are granted as needed, depending on the test features.

If you have a certificate of blindness and are able to take a test on computer, state this clearly in your application and specify what special accommodations you require.

In order to determine eligibility for special accommodations on the grounds of CP, you should submit a current medical assessment from a relevant specialist, such as a neurologist or rehabilitation doctor. The assessment should describe your medical condition and the manner in which it influences your functioning at this time in the areas relevant to the test. The determination of entitlement does not depend on the diagnosis itself, but on the actual effect of the limitation on performance in the test.

You can submit the request even if the doctor’s appointment has not yet taken place, and include the medical documentation that you possess. However, you should bear in mind that the determination of entitlement for special accommodations in the test is made on the basis of current and detailed medical documentation, and in many cases an assessment by a specialist is required. If the documents submitted do not included the required information, you may need to submit further documentations. This will incur an extra fee, and it may be performed until the end of the registration period for the test.
